NCI-H3122 Xenograft Model Service for NSCLC

Fig 1: NCI-H3122 xenograft model for NSCLC preclinical research.

The NCI-H3122 xenograft model, harboring the canonical EML4-ALK variant 1 fusion in an EGFR-wild-type, KRAS-wild-type background, represents the preclinical gold standard for evaluating ALK-directed tyrosine kinase inhibitors in lung adenocarcinoma. Alfa Cytology provides authenticated NCI-H3122 cell line-derived xenograft services with documented EML4-ALK fusion status, enabling robust preclinical assessment of first-, second-, and third-generation ALK inhibitors as well as combination strategies targeting acquired resistance.

Overview of NCI-H3122 Xenograft Model for NSCLC

NCI-H3122 was established from a pleural effusion metastasis of lung adenocarcinoma in a 54-year-old Caucasian male patient. The cell line carries the EML4-ALK variant 1 fusion (E13;A20), which results from the juxtaposition of echinoderm microtubule-associated protein-like 4 (EML4) exon 13 with anaplastic lymphoma kinase (ALK) exon 20, producing a constitutively active chimeric tyrosine kinase. Its wild-type EGFR, KRAS, and BRAF status isolates ALK as the sole dominant oncogenic driver, rendering the model exquisitely sensitive to ALK inhibition and an ideal platform for studying ALK-addicted tumor biology.

Iruplinalkib dose-dependently inhibited NCI-H3122 EML4-ALK xenograft growth with efficacy comparable to brigatinib and superior to crizotinib/ceritinib; intratumoral p-STAT3 suppression was observed in vivo but not in vitro, highlighting the H3122 CDX model's value for capturing tumor-microenvironment effects. (Yang, Yingying, et al., 2023)

When engrafted subcutaneously in immunodeficient mice, NCI-H3122 forms rapidly growing tumors with predictable kinetics, becoming palpable within 7–10 days and exhibiting robust dose-dependent responses to crizotinib. The model has been extensively validated in regulatory pharmacology studies, demonstrating inhibition of EML4-ALK phosphorylation, downstream STAT3/ERK/AKT signaling, Ki-67 proliferation markers, and induction of activated caspase-3 upon ALK inhibitor treatment. Consequently, the NCI-H3122 xenograft serves as an indispensable benchmark for evaluating novel ALK inhibitors, next-generation compounds overcoming gatekeeper mutations, and combination regimens that forestall acquired resistance.

Workflow of NCI-H3122 Xenograft Model Construction

NCI-H3122 xenograft construction follows a standardized protocol optimized for ALK-addicted epithelial cells, ensuring reproducible engraftment and robust pharmacological readouts aligned with regulatory pharmacology standards. The workflow proceeds through six integrated phases:

  1. Cell Expansion and Authentication: NCI-H3122 cells are propagated in RPMI-1640 with 10% FBS. Each batch is verified by STR profiling, mycoplasma testing, and EML4-ALK fusion confirmation by RT-PCR or targeted sequencing prior to implantation.
  2. Harvest and Matrigel Preparation: Adherent cultures are detached with trypsin-EDTA, washed, and counted. Cells are resuspended at 2 × 107 cells/mL in a 1:1 mixture of serum-free RPMI-1640 and high-concentration Matrigel to facilitate tumor take and stromal interaction.
  3. Subcutaneous Engraftment: A 100 µL suspension containing 2 × 106 viable cells is injected subcutaneously into the hind flank of immunodeficient mice. Tumor nodules typically become palpable within 7–10 days post-injection.
  4. Tumor Monitoring and Cohort Randomization: Tumor dimensions are measured twice weekly with electronic calipers, with volume derived from the formula length × width2 × 0.4. Upon reaching 100–150 mm3, mice are randomized into treatment groups stratified by tumor volume and body weight.
  5. Dosing and In-Life Assessments: Test compounds are administered via the designated route and schedule. Tumor volumes and body weights are recorded at defined intervals throughout the dosing phase. Optional blood draws support PK/PD correlation analysis.
  6. Terminal Procedures and Sample Processing: At study endpoint, tumors are excised, weighed, and imaged. Tissues are fixed in 10% neutral buffered formalin for H&E and IHC analysis, with portions snap-frozen in liquid nitrogen for downstream genomic or proteomic studies.

Case Study-NCI-H3122 Xenograft Model Development

In a preclinical program evaluating a novel second-generation ALK inhibitor, NCI-H3122 xenografts were established in athymic nude mice and dosed orally once daily for 21 days. The investigational compound produced dose-dependent tumor growth inhibition across multiple treatment arms, with the highest dose cohort demonstrating sustained suppression relative to vehicle-treated controls. Post-study immunohistochemistry revealed diminished phospho-ALK and phospho-STAT3 staining in responder tumors, alongside reduced Ki-67 proliferation indices and elevated cleaved caspase-3 expression indicative of apoptosis induction. These preclinical observations provided pharmacodynamic validation and supported the compound's advancement into extended tolerability and resistance mechanism studies.
