NCI-H2228 Xenograft Model Service for NSCLC

Fig 1: NCI-H2228 xenograft model for NSCLC preclinical research.

The NCI-H2228 xenograft model stands as a cornerstone reference for EML4-ALK fusion-positive non-small cell lung cancer, harboring the clinically relevant variant 3b rearrangement alongside a distinctive mixed adherent-suspension growth phenotype. Alfa Cytology delivers end-to-end NCI-H2228 xenograft services—from tumor establishment and longitudinal monitoring to resistance-clone profiling and pharmacodynamic biomarker analysis—empowering researchers to benchmark next-generation ALK inhibitors, dissect epithelial-to-mesenchymal transition-driven resistance, and evaluate rational combination strategies in a genetically authenticated, fusion-driven tumor setting.

Overview of NCI-H2228 Xenograft Model for NSCLC

The NCI-H2228 cell line was established in August 1989 from the primary lung tumor of a non-smoking female patient with lung adenocarcinoma. It represents one of the most extensively characterized models of ALK-rearranged NSCLC, distinguished by its expression of the EML4-ALK variant 3b fusion (E6;ins33;A20)—a chromosomal inversion on chromosome 2p that introduces a 33-base-pair insertion at the fusion junction, producing a constitutively active ALK tyrosine kinase with potent oncogenic capacity. Unlike the closely related variant 3a expressed in NCI-H3122, the NCI-H2228 variant harbors this additional insertion, which may subtly influence fusion protein stability, dimerization dynamics, and inhibitor binding kinetics. The line exhibits a unique mixed growth pattern comprising both adherent epithelial-like cells and floating suspension aggregates, a characteristic that demands adapted handling protocols but also offers a window into intratumoral heterogeneity.

In NCI-H2228 xenografts, the YHO-1701 (80 mpk) plus alectinib (4 mpk) combination achieved the deepest suppression of intratumoral survivin — the convergent downstream effector of ALK and STAT3 signaling — supporting STAT3 inhibition as a sensitizing strategy for ALK-TKI therapy. (Taniguchi, Keisuke, et al., 2021)

Beyond its defining ALK fusion, NCI-H2228 carries a homozygous TP53 p.Q331* nonsense mutation and an RB1 p.E204fs*10 frameshift alteration, creating a genomic background in which cell-cycle checkpoint control and p53-mediated stress responses are compromised. The line demonstrates exquisite sensitivity to first-, second-, and third-generation ALK tyrosine kinase inhibitors—including crizotinib, ceritinib, alectinib, and lorlatinib—with pharmacodynamic studies confirming potent suppression of phospho-ALK and downstream signaling. Notably, chronic ALK inhibition in this model can drive epithelial-to-mesenchymal transition as a resistance mechanism, while YAP1 transcriptional activation has been identified as a survival pathway that sustains tumor cells under alectinib pressure. These attributes render the NCI-H2228 xenograft an indispensable platform for profiling ALK inhibitor efficacy, mapping resistance trajectories, and testing sequential or combination therapeutic strategies in fusion-driven NSCLC.

Workflow of NCI-H2228 Xenograft Model Construction

Constructing a reproducible NCI-H2228 xenograft model demands particular attention to its mixed adherent-suspension growth phenotype and stringent quality controls to preserve EML4-ALK V3b fusion integrity throughout the study. At Alfa Cytology, all procedures are conducted under IACUC-approved protocols with documented standard operating procedures. The workflow is structured as follows:

  1. Cell Line Expansion and Pre-Implantation Validation: NCI-H2228 cells are propagated in DMEM/F12 (1:1) or RPMI 1640 with 10% FBS, with careful management of both adherent and suspension subpopulations. Each batch undergoes mycoplasma PCR testing, STR authentication, and EML4-ALK fusion verification by RT-PCR or FISH prior to harvest. Only cultures with confirmed fusion expression and >95% viability are approved for in vivo use.
  2. Recipient Animal Selection and Acclimation: Immunodeficient mice (BALB/c nude or NOD/SCID) are health-screened and acclimated for a minimum of five days. Baseline body weights and clinical observations are recorded to ensure uniform cohort health at study initiation.
  3. Subcutaneous Tumor Cell Implantation: A single-cell suspension (1 × 106 to 5 × 106 cells in 100–200 µL of serum-free medium, with or without Matrigel™) is injected subcutaneously into the flank under sterile conditions. Given the line's mixed growth properties, gentle dissociation is employed to preserve viability of both adherent and suspension fractions. Tumors typically become palpable within 10–14 days.
  4. Tumor Monitoring and Cohort Randomization: Tumor dimensions are measured by digital caliper every 2–3 days, with volumes calculated using the modified ellipsoid formula (length × width2 / 2). Upon reaching the target volume (typically 100–150 mm3), animals are randomized into vehicle-control and treatment arms using stratified randomization based on tumor size and body weight.
  5. Therapeutic Administration and Clinical Surveillance: Test compounds are administered according to sponsor-defined protocols (route, frequency, and duration). Mice are evaluated daily for body weight, clinical signs, and any treatment-related adverse events, with predefined humane endpoints strictly enforced.
  6. Terminal Harvest and Multi-Modal Endpoint Analysis: At study termination, tumors are excised, weighed, photographed, and allocated for comprehensive analysis: histopathology (H&E, Ki-67, TUNEL), ALK signaling profiling (p-ALK, p-ERK, p-S6 by IHC or Western blot), EMT marker assessment (E-cadherin, vimentin, N-cadherin), YAP1 localization and transcriptional activity, and pharmacokinetic correlation. All data are synthesized into a detailed study report with statistical evaluation.

Case Study-NCI-H2228 Xenograft Model Development

In a preclinical program focused on overcoming acquired resistance in ALK-rearranged NSCLC, Alfa Cytology established subcutaneous NCI-H2228 xenografts to evaluate the antitumor efficacy of a next-generation ALK inhibitor with enhanced blood-brain barrier penetration, both as monotherapy and in combination with a YAP1 pathway disruptor designed to counteract EMT-driven escape. Following tumor engraftment and randomization, animals received daily oral dosing over a four-week treatment window. Longitudinal caliper measurements demonstrated robust tumor growth inhibition in the monotherapy arm, with significantly enhanced response durability in the combination cohort. Terminal pharmacodynamic analysis revealed profound suppression of intratumoral p-ALK and p-S6 in treated tumors, while EMT markers remained attenuated in the combination group relative to monotherapy-treated animals. YAP1 nuclear accumulation was markedly reduced in combination-treated tumors, consistent with on-target engagement of the co-administered disruptor. Body-weight trajectories were stable across all cohorts, and no unexpected histopathological findings were observed in major organs. These findings highlight the translational utility of the NCI-H2228 platform for validating next-generation ALK inhibitors and rationally designing combination strategies to forestall EMT-mediated resistance in fusion-driven NSCLC.
